I’m frustrated

Frustrated by "Learning JavaScript" Example 13-1

I have worked through most examples in "Learning Javascript" (despite many typos), but Example 13-1 simply does not work for me. The example uses the XMLHttpRequest method and a php script to dynamically populate an option button, but no matter what I do it returns 'Error: preSearch Failed'. This is true both for the code typed in by me on the basis of the book, and for the sample code downloaded from the O'Reilly website.

I am testing this code in Safari on a mac.
