Channels & Masks PS extension from Deke McClelland's book not in the menu

I just bought Deke McClelland's book Channels & Masks one-on-one. It contains a bunch of installation instructions right in the introduction.
All is fine until I install the Channels & Masks panel. The Adobe Extension Manager accepts the installation and the extension seems properly installed.
However, after restarting PS, the new extension is not there. (Window -> Extensions -> a list but not Channels and Masks). Even restarting Windows doesn't help.

I use CS6, maybe that's the problem. Anyone else experienced the same? Or maybe Deke can help me out?

    I'm sorry to hear that you can't get that panel to work. The book was meant for Photoshop CS4, so I'm not surprised that the panel doesn't work. I checked through Deke's site to see if I could find any information on using it with CS6, but I didn't find any. I did find this note from Deke to folks that couldn't use the panel:

    "The good news is that the book doesn't hinge on your having the C&M palette up and running. I created it after the main body of the book was written, so with the exception of the Preface, the text does not specifically reference it. "
    ----
    So, you should be able to go through the book without it.

    Sorry I don't have a better solution for you.

    Thanks for your investigations. I understand that the book doesn't require a working C&M palette, so no big deal here. With or without the palette, the book is a great resource and a joy to read.
